List

A list allows for the display of standardized value choices. The different types of lists are:

• Dropdown list

• Radio button(single choice)

• Hierarchical list (exclusive to specialized form types like Professional Request)

• Check box (multiple choice)

• NP (single choice)

Creating a List

1. Create a list-type observation

2. Enter a title

3. Click the arrow to the right of the list observation to add list items

List Items

Items are the value choices displayed in the list.

1. Select “List Item”

2. Click the “Add Multiple” button

3. Add values by pressing "Enter" on your keyboard after each entry, then Confirm
